Medication Review
A CMR is a face-to-face (or phone-based) session between patient and pharmacist lasting 30 to 60 minutes. It is billable under Medicare Part D MTM programs.

Complete medication inventory
We gather every medication your patient takes: prescriptions, OTC drugs, vitamins, supplements. Often for the first time in one place.
Interaction & safety review
We screen for drug-drug interactions, drug-disease conflicts, inappropriate doses for age or renal function, and therapeutic duplications.
Adherence assessment
We identify which medications the patient is taking inconsistently or not at all, and why. Cost, side effects, and complexity are often the culprits.
Personal Medication List (PML)
The patient leaves with a printed, plain-language list of all their medications, what each one does, and how to take it.
Medication Action Plan (MAP)
A prioritized list of steps for the patient: what to discuss at their next appointment, what to watch for, what to stop taking.
Prescriber communication
We fax a clinical summary to the referring or primary care physician with our findings and any recommended medication changes.
Who qualifies for MTM?
Medicare Part D plans are required to offer MTM to high-risk beneficiaries. Beyond Medicare, any patient with a complex medication regimen can benefit.
